[Bug]: specify commands do not work for Codex CLI

Bug Description

I specify init in my project, and speck-kit md files were generated inside .codex but they're not visible in Codex CLI, why ?

commands should appear

commands do not appear in Codex CLI

BTW, they also don't appear in Codex app.. this is just broken
